2025 Cookie Program Parent Permission
Please complete this permission form for your Girl Scout to participate in the Girl Scout 2025 Cookie Program. Once the form is completed and submitted you will be emailed a copy of your submission. This email confirmation containing your submission must be provided to your Troop Product Coordinator. One form must be submitted per each Girl Scout.
